Requesting a NDIS plan reassessment

Circumstances can change and sometimes that means how we support ourselves needs to change too. If you receive NDIS funding, this can mean requesting a reassessment of your NDIS plan.

When is a reassessment recommended?

Reassessment of your plan is recommended if you have experienced changes in any of the following areas:

  • the impact of your disability on your life
  • a new life stage
  • living arrangements
  • caring supports.

How can I request a reassessment?

You can request a reassessment at any time if your circumstances change. 

To do this, contact the NDIS on 1800 800 110 or complete a change of details or change of situation form. This could result in a plan variation or plan reassessment.

In some circumstances, the NDIS reaches out a few months before the end of an individual’s plan for a ‘participant check-in’. You can alert the NDIS of your need for a plan reassessment at this time too.

What happens if I have requested a reassessment?

The NDIS will respond to your request for a plan reassessment within 21 days by either:

  • varying your plan
  • replacing your plan with a new plan
  • making a decision not to reassess your plan.
